Bridge Social Care & Quality of Life Coaching
Bridge is a specialist support and quality of life coach service for people with developmental disability or mental health condition. A hybrid service integrating 1:1 support and 1:1 neurodivergent coaching. We are person centred, we work closely with people to help them get the most from their circle of support. Through strength-based assessment we will learn significant life events and current circumstance, highlight existing skills and outline what would be required to achieve meaningful change for the people we support. From our assessment we develop a holistic care and support plan which aims to improve quality of life by increasing self-reliance, choice, control and meaningful occupation. Bridge staff are trained to support independent skill development and community participation. Bridge neurodivergent coaching takes advantage of this and coaching sessions can be used to work with people and their circle of support to co-produce care and treatment plans; reinforced by our Bridge staff supporting the person at home or in the community.
Our mission - To empower people to see beyond their diagnosis, their history or current situation. To see that they have the ability to change what they want in their life. To work in partnership with Integrated Care Board’s and NHS Trust’s to reduce the number of people experiencing delayed discharge due to a lack of specialist provision in the community. Caring is at the heart of what we do. We know living with a neurodevelopmental disability, a learning disability or mental health condition means that some days can be harder than others. That is why we always celebrate when we reach our goals and why we make time to rest and be kind to each other if things don’t go according to plan.

People should be in control of the care and support they receive. We educate people to become active members of their circle of support. Neurodivergent coaching helps people develop the skills and confidence to have agency over their care and treatment.

Life can be unpredictable, things don’t always go according to plan. We won’t panic, we will be determined to find the solution. We know anything can be achieved with the right amount of planning and support.
