Young people and adults (18+) living in London with developmental disabilities or mental health conditions and named on the Dynamic Support Register (DSR’s) of their local authority. Due to increasing and/or complex health and care needs that require extra support, care and treatment in the community as a safe and effective alternative to admission to a mental health hospital.

At risk of quality of life deterioration in the community owing to the current package of support failing to produce meaningful or sustainable outcomes.

Young people transitioning from children services to adult services.

Individuals returning home from hospital with a new or revised support package from community health team and/or social care services. Including those individuals discharged with a Community Treatment Order (CTO).

People we support have a lead professional who oversees their community support package either Care Coordinator, Social Worker or similar role.
