Outreach Support

  • Person centred care is at the heart of everything we do at Bridge. Our specialist outreach support services are tailored to meet the individual needs of the people we support. We work closely with individuals and their circle of support to co-produce care plans that are a true reflection of the wishes and needs of the individual.

    Support to access a meaningful program of activity with opportunities to develop independent living skills and build self-esteem.

    Developing practical life skills such as maintaining a healthy diet, managing finances, and food preparation.

    Assisting in finding employment, accessing education, and engaging in social activities.

    Managing personal and social relationships and addressing any arising difficulties.

    Navigating bureaucratic processes and understanding various official requirements, such as those needed for job interviews.

    Providing guidance on personal safety and protection from exploitation and abuse.

    Supporting personal development and emotional well-being, helping individuals feel empowered and confident in their independence.

    Active support and positive risk taking are fundamental in our care planning. We encourage community participation, we use risk assessments find the right balance between the wishes of the people we support and our safeguarding responsibilities. We work with health, education and social care colleagues to ensure our support plans are shared and understood, in the interest of creating safe and effective environments for the people we support.

    Bridge staff are trained in Positive Behaviour Support to improve quality of life and manage behaviours of concern. We put emphasis on providing proactive person centred care, using a consistent approach to communication and avoiding known triggers to reduce the need for behaviours of concern from people we support. All of the people we support have an individualised Positive Behaviour Support Plan (PBSP) developed and monitored by our Head of Complex Care and Specialist Support.

    We are committed to raising awareness and understanding of the strengths and needs of the people we support within their circle of support and their community.

  • Caring is at the heart of what we do. We know living with a neurodevelopmental disability, a learning disability or mental health condition means that some days can be harder than others. That is why we always celebrate when we reach our goals and why we make time to rest and be kind to each other if things don’t go according to plan.

  • People should be in control of the care and support they receive. We educate people to become active members of their circle of support. Neurodivergent coaching helps people develop the skills and confidence to have agency over their care and treatment.

  • Life can be unpredictable, things don’t always go according to plan. We won’t panic, we will be determined to find the solution. We know anything can be achieved with the right amount of planning and support.
