Bridge In-Reach Support plays a vital role in our commitment to NHSE Transforming Care Programme. As a provider of bespoke community support services it is our duty to work with those people who are experiencing delayed discharge due to a lack of specialist community support. We enable people to play an active role in planning their care and support from discharge through to independence. Ensuring the person we support has their views, wishes and dignity championed throughout.
Bridge In-Reach workers will visit as often as required to build rapport and trust. We want to ensure we have an understanding of what is important to the person we support. Bridge In-Reach Worker’s then link in with family, friends, ward staff, social care teams, community mental health teams and care coordinators. To develop person centred wrap-around care plans that enable better integration into the community. Ensuring progress and improvements are shared and celebrated. On hand to support monitoring and communicate relapse indicators, allowing for treatment to be arranged in the community to avoid readmission. Bridge In-Reach Program starts with the initial discharge planning meeting and continues to provide specialist community support from the point of discharge for up to 6 - 12 months as required.
Access to Bridge Neurodivergent Life Coaching sessions reinforces self-esteem and sense of purpose, adapting to change, treatment compliance and learning to live with a diagnosis.

Caring is at the heart of what we do. We know living with a neurodevelopmental disability, a learning disability or mental health condition means that some days can be harder than others. That is why we always celebrate when we reach our goals and why we make time to rest and be kind to each other if things don’t go according to plan.

People should be in control of the care and support they receive. We educate people to become active members of their circle of support. Neurodivergent coaching helps people develop the skills and confidence to have agency over their care and treatment.

Life can be unpredictable, things don’t always go according to plan. We won’t panic, we will be determined to find the solution. We know anything can be achieved with the right amount of planning and support.
